HEALTH CHECK

As part of the MOTIVATE Sefton we will ask you to complete a few measurements at home, to help keep track of your health.

Health Check Instruction Videos

As part of the MOTIVATE programme you will complete a health check using equipment sent to you in the post. When you receive the parcel complete the health check at a time convenient to you.
